Organization Overview

Since 1981 CHRIS 180 has been dedicated to healing and recovery for ren, youth, adults and families and is a growing $33+ million organization with over 385employees. CHRIS 180 services include: counseling for the community at large; school based mental health counseling; homes for abused, neglected foster ren with emotional and behavioral problems who are in state custody; supportive housing for single and parenting young people, ages 17 – 24 who are homeless or aging out of foster care; in-home services and community programs that strengthen families; outreach, drop-in and community housing for older teens, young adults and young adult families; a community center; an adoption program for ren in foster care; and, training for professionals, human service workers, educators, volunteers, individuals who work within the community, other organizations and ren, youth and families. The goal for all services is to unlock the potential that resides in each person to help them become self-sufficient, contributing adults who have the skills to navigate life’s challenges.

CHRIS 180 Re-entry program for the Fulton Re-entry Co-Occurring program utilizes a team approach with a licensed therapist and a Care Coordinator. The program targets 150 incarcerated males ages 18-24 in Fulton County Jail and are planning to return to the community. The position requires that part of the day is spent in Fulton County Jail for service provision of incarcerated individuals and the other part of the day in the community working with released individuals. The Reentry Case Manager assists participants in identifying and gaining access to medical, social, employment, educational, and other services which may impact recidivism. The Case Manager encourages the use of community resources through referral to appropriate traditional and non-traditional providers, paid, unpaid, and natural supports. Case Managers work in partnership with the therapist, individual and those identified by the client to play a role on the team. Case managers will provide a variety of skills-based training groups focused on reducing recidivism such as: Thinking for A Change, Life Skills, and Workforce Soft Skills Development Training.

Services are initially provided in-person in Fulton County Jail and they will continue in the community upon release. The Case Manager will provide group and one-to-one interaction with a designated enrollee; act as liaisons and advocates for the individual with other agencies as needed, complete required paperwork, and perform other related duties as required. Case Managers must be flexible in working hours, be able to respond to crisis situations in an effective manner and meet regularly with their supervisor.

Role and Responsibilities

The primary duties include but are not limited to the following:

This position provides supervision to residents/member as well as administrative responsibilities around incident reporting, case notes and medical appointments. Primary duties include but are not limited to the following:

Case Management Specific Skills

  • Provide case management for those enrolled in re-entry programming.
  • Group skills training include but not limited to: Thinking for A Change, Life Skills, and Workforce Soft Skills Development Training.
  • Demonstrate a sense of urgency and utilizing engagement skills and wraparound knowledge to target client population.
  • Maintains a caseload of up to 25 individuals (both incarcerated and released)
  • Establishes contact upon initial release from jail (24 hours) and meet face-to-face within 48 hours
  • Completes case notes within 48 hours of contact with the consumer in electronic health record.
  • Completes a comprehensive intake assessment of the participant to determine services needed, including activities that focus on the need for any medical, educational, employment, identification needs, risk management, mental health needs, substance misuse needs, social or other service are needs.
  • Assess client history of trauma, strengths and needs;
    • Aware of ACE score and implications on service planning
    • Pre and post tests for all groups facilitated
    • Risk Assessment (ACTs)
  • Develops an Action Plan or Release Plan that specifies the goals of providing care management and the actions to address identified needs.
    • Provides resources and referrals to the individual accessing services and supports as identified as a need prior to release.
    • Ensures that the services being provided are responsive to the client needs identified in the plan and that the client is experiencing voice, access and ownership of their plans and services
    • Communicate with service providers to overcome barriers within the action plan. If direct communication is not effective, the Case Manager will address ongoing issues with supervisors.
    • Assessment of the strengths and needs and promotes strategy development with assistance from client and team members
  • Ensures that ongoing meetings are held every 30 days at a minimum to gain understanding of ongoing needs and updates on outcomes
    • Monitors the provision and quality of services through the team and modify the Action Plan per the guidelines above.

Job Development Specific Skills

  • Facilitate work readiness curriculum for clients to gain soft skills related to future employment including interviewing, resume preparation, and other skills
  • Interview client relative to placements being sought
  • Ensure clients are prepared before going to job interviews, and conduct follow-up assessments
  • Develop and maintain partnerships in the public and private sectors that will provide career opportunities that are aligned with populations’ interests and goals
  • Identify and schedule career-related guest speakers, site visits, job shadow experiences, and informational interviews
  • Assist with post release job placements by connecting clients with viable job opportunities
  • Work closely with employers of clients to maximize employment retention and success
  • Provide evaluation, conciliation, and mediation between employer and employee post job placement
  • Conduct bi-weekly case management meeting with clients to develop, plan and track progress towards goals regarding completion of GED, obtaining stable employment and/or entering higher education
  • Other duties as assigned

Qualifications/Competencies

Minimum: Bachelor’s degree in one of the social sciences and experience working with at-risk population.

Preferred: The above minimum qualifications and experience working with incarcerated or prior incarcerated individuals, individuals with a history of substance and/or mental health diagnosis

  • Must have the ability to establish and maintain professional relationships with individuals in the community and professional agencies
  • Ability to interact with various client populations.
  • Ability to express ideas effectively and professionally both verbally and in writing.
  • Must have a dependable vehicle since the position requires travel and insurance coverage beyond minimum liability.
  • Exhibit excellent organizational skills to deal with scheduling, budgeting, coordinating services, etc.

Schedule

Work Hours (an overall general statement): This is a full-time position. In general terms, the position works M-F 9:00am-5:00pm. Shift starts in Fulton County Jail promptly at 9:00am
